Fall Prevention Partner Program Information
This fall prevention program is all about health care partners throughout the community to incorporate fall prevention screening as part of their plan of care. We provide the screening tool and for those who answer yes to more than one questions who are determined to be a fall risk. We then provide a free home safety assessment performed by our physical and/or occupational therapist. Please contact us today to learn more. Your patients health and safety is worth it!
